What it is

A CPST worker helps you put your plan of care into practice: appointments, medication follow-through, benefits, housing, school, work, and community resources.

Who it helps

Children, adolescents, and adults who need support carrying a plan of care into everyday life.

What to expect

Regular meetings, often in the community or your home, focused on practical goals you choose.

How it may help

  • Fewer missed appointments and gaps in care
  • Connection to community resources
  • Steadier daily routines and independence

How care is delivered

Available in home, community, in office, telehealth, depending on your needs, location, and what is clinically appropriate.

General eligibility

Services begin with a behavioral health assessment, which helps determine whether this service fits your needs. We serve Ohio residents ages 5 and up.

Insurance considerations

We accept Ohio Medicaid and Medicaid Managed Care Plans, Medical Mutual, and Medicare. Coverage is always subject to verification. Benefits depend on your specific plan, medical necessity, provider eligibility, and plan requirements. Our office will confirm your coverage before services begin.

How to get started

Call our office or send a short message and we'll schedule a behavioral health assessment. No outside referral is required — and professionals can refer a client from our referral page.
