Behavioral Health Assessment

A comprehensive clinical assessment that guides the right plan of care.

In officeTelehealth

What it is

A conversation with a clinician about what you're experiencing, your history, and your goals, so care can be matched to your needs from the start.

Who it helps

Anyone beginning services with us — children ages 5 and up, adolescents, adults, couples, and families.

What to expect

One appointment, usually the first one. We ask questions, listen, answer yours, and then recommend the services that fit. Nothing is decided without you.

How it may help

  • Understanding what you're experiencing
  • Getting a clear, written plan of care
  • Being matched with the right clinician and service

How care is delivered

Available in office, telehealth, depending on your needs, location, and what is clinically appropriate.

General eligibility

Services begin with a behavioral health assessment, which helps determine whether this service fits your needs. We serve Ohio residents ages 5 and up.

Insurance considerations

We accept Ohio Medicaid and Medicaid Managed Care Plans, Medical Mutual, and Medicare. Coverage is always subject to verification. Benefits depend on your specific plan, medical necessity, provider eligibility, and plan requirements. Our office will confirm your coverage before services begin.

How to get started

Call our office or send a short message and we'll schedule a behavioral health assessment. No outside referral is required — and professionals can refer a client from our referral page.

In a crisis? Call or text 988 for the Suicide & Crisis Lifeline, or call 911 in an emergency. Our office, website, and forms are not emergency services and are not monitored around the clock.
