Community-Based Services

In-home, school-based, and community support when appropriate.

In homeSchoolCommunity

What it is

Care delivered outside our office — in your home, at school, or in the community — when that is where the support is most useful.

Who it helps

Youth and adults whose goals are best practiced in real settings, and families for whom coming to an office is a barrier.

What to expect

A provider travels to you. Availability depends on location, schedule, and clinical appropriateness — call us and we'll tell you honestly.

How it may help

  • Support in the setting where challenges happen
  • Fewer transportation barriers
  • Closer coordination with schools and caregivers

How care is delivered

Available in home, school, community, depending on your needs, location, and what is clinically appropriate.

General eligibility

Services begin with a behavioral health assessment, which helps determine whether this service fits your needs. We serve Ohio residents ages 5 and up.

Insurance considerations

We accept Ohio Medicaid and Medicaid Managed Care Plans, Medical Mutual, and Medicare. Coverage is always subject to verification. Benefits depend on your specific plan, medical necessity, provider eligibility, and plan requirements. Our office will confirm your coverage before services begin.

How to get started

Call our office or send a short message and we'll schedule a behavioral health assessment. No outside referral is required — and professionals can refer a client from our referral page.

In a crisis? Call or text 988 for the Suicide & Crisis Lifeline, or call 911 in an emergency. Our office, website, and forms are not emergency services and are not monitored around the clock.
